PAD Risk Self-Assessment

Peripheral Arterial Disease (PAD) affects more than 8 million Americans and is often silent until the disease is advanced. Answer 8 quick questions to estimate your risk and learn whether a diagnostic ultrasound is recommended.

This self-test is educational and is not a diagnosis. If you have severe leg pain, a non-healing wound, or sudden coldness in a limb, call your doctor or 911 immediately.

1. What is your age range?
2. Do you currently smoke, or have you in the past 10 years?
3. Do you have diabetes?
4. Do you have leg pain or cramping that comes on with walking and improves with rest?
5. Do you have a wound or sore on your foot or lower leg that is healing slowly or not at all?
6. Have you been told you have high blood pressure or high cholesterol?
7. Personal history of heart attack, stroke, or coronary artery disease?
8. Does one foot or leg feel noticeably colder than the other, or appear paler/bluer?
